Report A Claim Monday through Friday from 8:00 a.m. to 5:00 p.m. call 815-398-6800 and ask for the Claim Department. After hours the claim may be reported direct to your insurance carrier listed on the 24/7 Claim Contacts.

In the event of a catastrophic claim, call 815-398-6800 and follow the prompts to speak with one or our Claims Customer Service Representatives.
